Add a customer feedback form to your website

Describe the questions and Siter AI builds the feedback form. Visitors rate with stars, pick what they liked, add a comment, and send it to you.

A restaurant's feedback form as a visitor sees it: the question How was your visit, five stars with four selected, the chips Food, Service, Atmosphere and Value, a comment box with a 300 character counter and a Send feedback button.

A feedback form that looks like your page.

  • Stars, chips and a comment

Visitors rate with stars, tap what they liked, and add a comment if they want to.

  • Feedback comes to you

When a visitor sends feedback, the rating, the chips and the comment are saved and emailed to you.

    2 places the feedback lands

    • Form entries
    • Your email
  • Takes your fonts and colors

The form is part of the page, so it wears your brand colors, text color, background and corner radius.

Describe it, then build it.

Place an App, say what visitors can rate and pick, and Siter AI writes the form, tests it and shows it on your canvas.

  1. What should the app do?

Say it in a sentence, with the stars, what visitors can pick, such as Food or Service, and whether they can leave a comment.

  2. Or start from one of these

The starters cover calculators, a quiz and more. A feedback form is not on the list, so you describe it in the box above.

  3. Build

Siter AI writes the form and tests it, fixing an error if it finds one, while you watch the progress.

  4. Try it on the canvas

The form runs in its own sandbox, apart from your website. Double-click it to send yourself feedback.

What your visitor does.

  • Rates with stars

One to five stars, right on your page, in a tap.

  • Picks what they liked

Chips you choose, such as Food, Service, Atmosphere or Value, for example.

  • Adds a comment

A short note, optional, and a name and email if you ask for them.

  • Sends it to you

It lands in your Form entries and your email. You can write a different thank-you for a high rating and a low one. The form does not show reviews on your page.

From idea to live in three steps.

  1. Place an App

Pick App in the left toolbar and put it on the page where the feedback form belongs.

  2. Describe your feedback form

Write what visitors rate and pick, whether a comment is optional, and press Build.

  3. Publish and send a test

Publish the page, send yourself feedback, and check that it arrives in Form entries and your email.

How do I add a feedback form with a star rating to my website?

Place an App from the editor's left toolbar and describe the form: the stars, what visitors can pick and whether a comment is optional. Siter AI writes it and tests it, and you publish the page like any other. It is not an embed or a code snippet.

Where does customer feedback go?

To Form entries and to your email. When a visitor sends feedback, it is saved in your website's Form entries and emailed to you.

Does the form show reviews on my website?

No. It sends feedback to you and does not publish reviews or connect to a review site. What you do with the answers is up to you.

Is this a feedback form template, and can I use it for an event or a client?

There is no fixed template. Siter AI builds the form to your description, so the questions are yours, for an event, a client or your customers. A typical description asks for 1 to 5 stars, a few chips and an optional comment.

How is this different from the forms on a Siter website?

Website forms cover contact, signup and other forms in general. The feedback form is an App built to your description, with a star rating and chips. For appointments, see the booking form.

Which plan do I need for a feedback form?

You can build and preview a feedback form on any plan, including Free. An app shows on your published website on paid plans, and each plan includes its own number of apps per website. See the plans.
